Examination/Application Information
The prescribed application form and filing procedures for examination as Professional Geologist have been revised.
The revised application form supersedes all previous versions. Applicants and reference providers should not use any prior version and should dispose of any in their possession.
All applications and the required supporting documents must be submitted by the final filing date.
Once the Board has approved an application the applicant will not be required to submit new reference forms for two years.
All applications require the $250 application fee, including those filing to take an examination that was previously failed.
Examination fees are specified in the fee schedule for licensees and applicants. Applications to take an examination that was previously failed also require payment of the the examination fees as specified in the fee schedule. All fees are due at the time the application is received.

The Application Form is fillable .PDF forms. The latest version of the FREE Adobe Acrobat Reader should be installed to complete the forms.
We suggest that you print a copy and complete it by hand. When you're sure all the answers are complete, open the online file again and enter your responses on the form, print it, sign it, and mail the original with all required documentation. Be sure to make a copy for your own files.
